In one case, a patient received neomycin/polymyxin B/ hydrocortisone OTIC (ear) suspension instead of OPHTHALMIC (eye) suspension. This resulted in severe burning and eye pain after use.
In other situations, carbamide peroxide drops for earwax were inadvertently used in the eye...causing stinging, swelling, and redness.
