Stop Mix-Ups With Oral Combo Meds in Their Tracks

Mix-ups with combination meds are back in the news.

In a recent case, an order was written for Entresto (sacubitril/ valsartan) 100 mg. The pharmacy dispensed Entresto 97 mg/103 mg...thinking it was closest to the 100 mg strength prescribed.

But it turns out that the order was supposed to be for Entresto 49 mg/51 mg...since adding 49 mg and 51 mg equals 100 mg.
