Promote Safe Transitions of Care With HIV Meds

Patients with HIV are at high risk for med errors during an admission.

HIV patients usually take at least 3 antiretrovirals, such as dolutegravir, tenofovir, and emtricitabine...as a combo or separate pills. But errors can lead to resistance, treatment failure, or adverse effects.

Be aware that abbreviations are often used for antiretrovirals...such as “TAF” for tenofovir alafenamide and “TDF” for tenofovir disoproxil fumarate. Clarify these if necessary to avoid mix-ups.
