Revisit Safeguards for Keeping Vials Straight

You’ll see more focus on avoiding vial mix-ups...due to a warning on cisatracurium vial caps that’s hard to see.

This cisatracurium product is 20 mg/10 mL from Teva.  The words “Warning: Paralyzing Agent” are in black letters on a dark blue cap...instead of the usual white lettering.

It’s scary because these could be mistaken for other similarly sized vials with dark blue caps (lidocaine, etc)...and accidentally injecting a neuromuscular blocker is deadly.
