Use Clear and Complete Information to Prevent Med Name Mix-Ups

Shortened versions of med names will continue to cause errors.

In a recent case, use of "PCC" on an order led to a patient getting prothrombin complex concentrate (Kcentra, etc), a med that STOPS bleeding...instead of protein C concentrate (Ceprotin), a med that may CAUSE bleeding.

Avoid using med name abbreviations and clarify when others use them. Don't just avoid your pharmacy's list of dangerous abbreviations...they're sometimes added only AFTER there's been an error.
