You may see orders for "fixed" doses of Kcentra...to emergently reverse the effects of warfarin.

The usual dose of this prothrombin complex concentrate is 25 to 50 units/kg...depending on how high warfarin has increased a patient's INR.

But giving a fixed 1,000 or 1,500 unit dose can simplify ordering and prep...and help patients get faster treatment if their INR isn't known.
